Heather at Beautiful Buttons makes button templates and takes requests. When I visited her site it was very organized and has button templates for anyone! Heather has a huge selection!

She usually gets them done the day they're requested and she can do pretty much anything with buttons including animations. She also is willing to do something pixel by pixel for someone if that might be the case. she is great to the people that visit her site
